NURSE FOR CHANGE MOVEMENT TO HOLD A "MASS ACTION"

Nurse for Change Movement is set to conduct a protest on October 17, 2014 against the government after the Department of Budget and Management (DBM) denied the proposed wage increase for Nurses last May. In a letter dated May 26, 2014 by DBM to Rep. Leah S. Paquiz, the department apparently turned down the proposed increase in wages of nurses.
Dr. Carl E. Balita, a recognized registered nurse and a radio commentator, called for nurses through social media to take action and conduct a “mass action,” or remonstration on October 17, 2014.

Balita, initiated to create Nurses for Change Movement with the help of other Nurses organizations and groups. Their advocacies are salary increase for nurses, stop false volunteerism, and have adequacy nurse-patient ratio.

Meanwhile, the Nurses for Change Movement is on the process of generating more supports for the cause.

Registrants are advised to download the Oath Forms HERE and to bring their duly accomplished Oath Forms during their schedule for registration. Registrants are also advised to strictly observe the schedule for initial registration to avoid any inconvenience.

PRC INITIAL REGISTRATION FOR NEW NURSES

Initial registrants are advised to observe the following steps:

  1. Download your oath form HERE and bring the following:
    1.1 Printed Oath Form. Please note that there is NO NEED to have the Oath Form notarized.
    1.2 One (1) documentary stamp
    1.3 One (1) passport size picture in plain background with complete name tag (for Registry of Professionals)
    1.4 Current Community Tax Certificate (cedula) 
  2. Present duly accomplished or downloaded oath form at designated counter/window (verification officer) for assessment of prescribed fees.
  3. Proceed to the cashier window to pay the prescribed fee. Cashiers’ windows are located at the 3rd, 4th, and 5th floors of the PRC Annex Bldg and/or at the ground floor of the PRC Main Bldg.. 
  1. After payment, proceed to the designated Registration counter/window. The Registration Officer will print Registry Sheet with assigned Registration / License Number to be signed and thumb marked by the Registrant.

  2. Upon completion of the Registry Sheet, issuance officer shall issue claim slip/stub.
  3. Receive claim slip/stub and claim your Professional Identification Card as scheduled indicated on your claim slip/stub.

Important Notices!

In case registrant is located abroad, please find below the following requirements:
  1. Take the professional's oath before the Philippine Consul.
  2. Special Power of Attorney notarized by the Consul.
  3. Copy of passport page showing personal data.
  4. Copy of passport page showing date of entry abroad

Renewal of Professional ID

In line with the new online registration system in place starting July 18, 2012, professionals renewing their professional identification cards (PICs) are advised of the following revised procedures:

  1. RENEWAL AT PRC CENTRAL AND REGIONAL OFFICES
    1. Professional or duly authorized representative must secure and accomplish a renewal form from the Registration Division and submit one (1) passport size picture with nametag and in white background.
    2. Professional pays to cashier the renewal fee upon securing his/her printed renewal application. The schedule for claiming the PIC is printed on the lower portion of the printed form.
    3. On the scheduled date, the professional or his/her duly authorized representative must present claim stub to the Registration Division. He/she shall also present the Official Receipt showing payment of the renewal fee and sign an acknowledgment of receipt of the PIC. When the PIC is claimed by a representative, the said representative must also present together with the official receipt a *letter of authority or **Special Power of Attorney from the professional.

  2. RENEWAL AT SM MALLS
    1. Professional fills out renewal form at Business Center of any SM Mall.
    2. He/she pays assessed fee for renewal, photo fee, convenience fee and/or courier service fee, the latter if he/she desires to have the PIC sent through courier to his/her address. Please note that the photo fee, convenience and/or courier fee are payments to SM Mart, Inc. and do not form part of PRC's fees.
    3. A professional not availing himself of courier service must claim the PICs on the date indicated in the claim stub. When the PIC is claimed by a representative, the said representative must also present together with the official receipt, a *letter of authority or **Special Power of Attorney from the professional.

Professions to submit an updated certificate of good standing from their respective Accredited Professional Organization (APO) are the following:

     **Representatives filing the application and claiming the professional ID in behalf of the professional must present Special Power of Attorney and valid identification of the professional and the representative.

The Professional Regulation Commission (PRC) “Panunumpa ng Propesyonal (Oath of Professionals)” form is a government form which is free to obtain, print and/or reproduced. This form is available in the PRC office but you may also download it from here. In order to save more time, it’s better to print out a copy of this form and fill it out before going to the PRC office.

OATH TAKING CEREMONY FOR NEW PROFESSIONAL NURSES HELD


The Board of Nursing administered the Oath of Professionals to successful passers of July 2010 Nurse Licensure Exam (NLE) which was administered in 15 testing centers throughout the country. The event was held on September 20-21, 2010 at the SMX Convention center beside Mall of Asia, Pasay City. The said ceremony was chaired by Honorable Leonila Faire, member of the Board of Nursing, Professional Regulation Commission. 

Sor Paz Marfori, a recipient of the Most Outstanding Nurse award for the year 2010 was the Keynote Speaker but was not able to be present on the said affair due to some health reasons. The PRC chair Hon. Nicolas Rapena Jr., gave his message to inspire the new nurses. Furthermore, Dr. Carmelita Divina Gracia, president of the ADCPN, committed the new nurses to the public as they legally commence their practice of safe and quality nursing. The new successful nurses were also inducted to the Philippine Nurses Association by Dr. Teresita Barcelo, PNA president.

The top ten examinees and the top performing schools were also given recognition.

PROCEDURES AND REQUIREMENTS DURING THE PRC INITIAL REGISTRATION

PROCEDURES
1.      Accomplish this Oath Form.
2.      Pay for two sets of metered documentary stamps (one stamped on this form and one for the Registration Certificate) at the Customer Service Center.
3.      Present duly accomplished oath form (where your profession belongs, for verification of records and assessment at Window 14, Window 19,Window 28 or at PRC Service Center, Ever-Recto
4.      Pay prescribed fees at the Cashier.
5.      Sign in the Registry Book and receive your claim slip at: Window 17 Window 20 Window 27
6.      . Claim your professional ID as scheduled. Please refer to your claim slip for further instructions.

REQUIREMENTS
1.      Duly accomplished Oath Form or Panunumpa ng Propesyonal
2.      Current Community Tax Certificate (cedula)
3.      Two pieces passport size colored picture in white background with complete name tag (for Oath Form and Certificate of Registration)
4.      One pc. 1x1 colored picture in white background with complete name tag (for Registry of Professionals)
5.      Two sets of metered documentary stamps (for Oath Form and Certificate of Registration)
6.      One (1) short brown envelope with name and profession

New registrants should come personally to the PRC Central Office or to the nearest Regional Office to file his/her application for registration and to affix his/her signature on the registry book.

In case registrant is located abroad, please find below the following requirements:

1.      Take the professional’s oath before the Philippine Consul.
2.      Special Power of Attorney notarized by the Consul.
3.      Copy of passport page showing personal data.
4.      Copy of passport page showing date of entry abroad.

Board Certificates shall be claimed at the Central Office or the Regional Office where the application for the same has been filed. Representatives must present Special Power of Attorney and appropriate identification papers.
